If there is a regulatory concern, KOEC is required to conduct an on-site visit, also called a complaint survey, to determine if the licensed child care facility is in compliance with regulations. Concerns reported about someone providing child care without a license are also investigated and involve an on-site visit by a child care surveyor. Once a complaint is submitted it cannot be "withdrawn."

Child Care Complaint Form

This form, for License and Group Day Care Homes, Child Care Centers and Preschools, and School Age Programs is to be used when reporting an injury, death, critical incident or occurrence that jeopardizes the safety of any child in care pursuant to K.A.R. 28-4-133 and K.A.R. 28-4-592. This form may be used for Drop in Programs reporting to KOEC.

Please utilize this form to notify KOEC of the closure of a child care facility. Do not complete this form until the facility has closed and is no longer providing services.
